Basketball Lineup Sheet for the Scorebook (Free Printable)
A basketball lineup sheet is the one-page list a coach hands the scorer's table before a game: the team name, every player's name and jersey number, the five starters marked, and the head coach's signature. It exists because the rulebook puts a clock on it. Under NFHS rules 3-2-1 and 3-2-2, each team must supply its roster and designate its five starters in the official scorebook at least ten minutes before the scheduled start, and a list that arrives late, changes a number, or swaps a starter after that point costs the team a technical foul. The sheet on this page prints two copies per page so the home book and the visiting book get the same list, plus a roster-only variant, and the Excel file fills both copies from a single roster list.

Page one holds two identical copies, labeled Home Book and Visiting Book, each with team, opponent, date, site and level, 14 player rows with number, name, starter and captain marks, the jersey-number rule, the head coach signature line and a time-given-to-scorer box. Page two is the roster-only variant for leagues that do not designate starters.

Type the roster once on the Roster tab, mark the five starters and the captains, and the Home Book and Visiting Book copies fill themselves; a check cell flags more or fewer than five starters, a duplicate number, or a number that uses a digit above 5.
Both files are free, with no email or account. For Google Sheets, upload the Excel file to Drive and open it; the formulas carry over.

What Goes on the Sheet
Only what the scorer needs to open the book. Team and opponent, date, site and level go in the header so the sheet can be matched to the right page in the right book. Each player row carries the jersey number first, because the scorer enters players in number order and reads numbers, not names, during the game; the name follows in last-first order to match the book. A starter mark goes beside exactly five players. A captain mark is optional at most youth levels but worth keeping, because officials speak to captains during the game and the scorer records them in the book at many schools.
Two things on the sheet are rules, not conventions. The jersey numbers must match what the players are wearing, and under NFHS 3-4-3 a legal number uses only the digits 0 through 5, as one or two digits, with 0 and 00 treated as the same number so a team may not list both. The head coach signature line matters because the coach, not the scorer, is responsible for the accuracy of the list; the scorer transcribes it.
Why Two Copies
At a high school game there are two books: the home team's book is the official one (NFHS 2-11-1 names the official scorer, who is normally provided by the home team), and the visiting team keeps its own. Both need the same roster and the same five starters, which is why the sheet prints as two identical copies with a cut line. Hand one to the official scorer and one to the other bench's scorekeeper, and when the two books disagree later, the lineup sheet is the shared starting point. At a youth league game with one book, the second copy is for your own bench, so the assistant tracking minutes on the rotation chart is working from the same numbers the table has.
The Ten-Minute Routine
The deadline is the scheduled start, not the actual tip, so the routine starts the moment the team arrives.
- Fill the sheet in the locker room or on the bus, including starters. Deciding starters at the table is how a team ends up with six marks.
- Walk both copies to the scorer's table at least ten minutes before the scheduled start. Say the starters out loud while the scorer marks them; the spoken check catches a wrong number faster than a second read.
- Confirm the scorer wrote the time in the "time given to scorer" box. If a dispute comes later about whether the list was on time, that box ends it.
- Check that every number on the sheet is on a jersey in the layup line. A player who changed jerseys since the roster was typed is the most common cause of a number penalty.
- Confirm the possession arrow and the game clock are set, then leave the table alone until the horn.
What the Penalty Actually Is
The NFHS penalty structure is specific and worth knowing before you argue about it. Failing to supply the roster and starters by the ten-minute mark is a team technical foul. After the deadline, adding a name to the list, changing a number, or changing a designated starter is also charged as a technical, with a maximum of one administrative technical per team for these roster and starter infractions (NFHS 10-1-1 and 10-1-2). A starter may be changed without penalty only in the cases the rule lists: a starter who is injured or ill, or a substitution needed so the right player attempts a free throw for a technical foul assessed before the game; the book must be told either way. The opponent shoots the free throws for the technical and gets the ball at the division line, and the technical counts toward the team-foul total for the quarter, which can put the other team in the bonus before the first field goal. The scorebook guide covers how the scorer records it.
Youth League Variants
Most rec and travel leagues borrow the NFHS shape without the full penalty. Common variants: no signature required, starters not designated because the league plays segment or equal-time rules, a maximum roster size printed on the sheet (10 or 12 is typical), and an eligibility line, since many leagues require that a player appear on the lineup sheet to be eligible to play that day, which means a late-arriving player who is not on the list sits. If your league enforces minimum-play rules through the book, the lineup sheet is also the participation record: the scorer marks each listed player's first entry, and a name missing from the sheet cannot be credited. The playing time guide covers how those rules are written and how to plan around them.
From the Lineup Sheet to the Rotation
The lineup sheet answers who may play; it says nothing about how long. The free basketball rotation generator takes the same roster, with the five starters marked, and builds the shift-by-shift substitution plan for your game format, equal time or starters-weighted, then prints the rotation card the bench runs from. Rizzler does not print this NFHS lineup sheet from Game Manager today; the download on this page is the sheet for the table, and the rotation card is what the product prints. Save the plan to a free account and the planned minutes from every game collect in Game Manager's playing-time view.
Frequently Asked Questions
Who gets the basketball lineup sheet?
The official scorer, at the table, at least ten minutes before the scheduled start. At a two-book game, give the second copy to the visiting scorekeeper so both books open with the same roster and starters; at a one-book youth game, keep the second copy on your bench.
What if a player arrives after the ten-minute deadline?
If the player's name and number were on the sheet, nothing happens; being on the list is what the deadline covers, not being in the gym. If the name was not on the sheet, adding it after the deadline is a technical foul under NFHS rules, and some youth leagues bar the player for the day instead. Put every rostered player on the sheet whether or not you expect them, and let a no-show be a no-show.
Can I change a starter after turning in the sheet?
Yes, with a technical foul, unless the change is for a reason the rule lists (an injured or ill starter, or a free throw for a pre-game technical), in which case the book is simply told. The limit is one administrative technical per team for roster and starter changes combined.
What jersey numbers are legal in high school basketball?
Under NFHS 3-4-3, numbers use only the digits 0 to 5, as one or two digits: 0 through 5, and 00, 10 through 15, 20 through 25, and so on up to 55. A team may not have both 0 and 00. Youth leagues often allow any number, but the official book at a school game will not.
Does Rizzler print a basketball lineup sheet?
Not this sheet. Game Manager for basketball prints the rotation card (the shift-by-shift lineups and planned minutes from a saved plan). The NFHS-format roster and starters sheet is the download on this page; use the Excel file if you want it filled from a typed roster.
Is the lineup sheet the same as a score sheet?
No. The lineup sheet is handed in before the game and lists who may play and who starts. The score sheet is kept during the game and records the running score, points, fouls and timeouts. The scorer copies the lineup sheet into the score sheet or book before tip-off.
